Set up the Permission to Contact form

Configure your Permission to Contact (PTC) form, a Medicare consent form a beneficiary signs to allow you to contact them, before you share its link with anyone. This is separate from your capture form. The capture form collects a name, email, and phone number. The PTC form records specific, detailed permission to be contacted about Medicare products.

Before you start

Filling in this page is available on every plan. There’s no plan check to open or configure it. Only getting a usable, shareable public link is gated to paid plans. See “Plan gating” below.

Read this warning before you do anything else. It appears at the top of the Permission to Contact page every time you open it, and it does not go away on its own:

This form has not been reviewed by counsel or approved by your carriers. Do not enable it until both are complete.

Do not activate this form until your own legal counsel has reviewed it and your carriers have approved it. This page explains how to fill in the settings. It does not tell you when it’s legally safe to turn the form on.

Steps

1. Open Permission to Contact settings

Select Form settings on the PTC Consents page, or select Permission to Contact settings from the “Add a link” dialog when you’re editing a post (see Collect leads from your posts for how to open that dialog). Both links go to the same page.

2. Fill in your agency and agent details

Under Agency and agent, enter:

  • Agency legal name
  • Doing-business-as name (optional)
  • Licensed agent’s full name
  • National Producer Number (NPN): digits only, up to 10 digits
  • Agent phone number
  • Agent email (optional)
  • Service area: for example, “Wake County, NC”
  • I represent every plan available in this service area: check this box if it’s true for you.
  • Organizations represented: the number of organizations you represent in this service area.
  • Products offered: the number of products you offer in this service area.

3. Choose the coverage types you offer

Under Types of coverage you offer, check every type that applies:

  • Medicare Advantage (no drug coverage)
  • Medicare Advantage with prescription drug coverage
  • Standalone Medicare prescription drug plan
  • Medicare Supplement (Medigap)
  • Dental, vision, hearing

You must check at least one.

4. List your data-share recipients

Under Data-share recipients, select + Add recipient for each organization you share signed permissions with, and enter that organization’s name.

Each recipient must be a specific legal entity. The page states this rule directly:

Select Remove next to a recipient to take it off the list.

5. Confirm your branding

Check I confirm the logo and brand name on this form are my agency’s own, not a carrier’s. This form must be carrier-neutral: it cannot show a carrier’s name or logo anywhere, and it cannot be limited to one carrier’s products. If a carrier name is detected anywhere in your agency name, DBA name, service area, or data-share recipients, you cannot activate the form until you remove it.

6. Review the activation blockers

If anything required is still missing, a red Resolve these before you can activate this form: box lists each one in plain language above the form fields. The possible messages are:

  • Enter your agency’s legal name.
  • Enter the licensed agent’s full name.
  • Enter a valid National Producer Number (1–10 digits).
  • Enter a phone number beneficiaries can reach you on.
  • Choose at least one type of coverage you offer.
  • Enter how many organizations you represent in this service area. The required disclaimer states this number.
  • Enter how many products you offer in this service area.
  • List each recipient’s legal entity name. Categories such as ‘our marketing partners’ are not permitted.
  • Remove the carrier name. This form must stay carrier-neutral — naming a carrier limits the permission to that carrier’s products.
  • Confirm the logo and brand name on this form are your agency’s own, not a carrier’s.

Fix everything the box lists before you try to turn the form on.

7. Activate this form

Check Activate this form, then select Save.

If every blocker is already resolved, the box below the checkbox doesn’t appear and saving turns the form on. If blockers were still on file as of your last save, you’ll see this note next to the checkbox:

A successful save shows Saved briefly next to the Save button.

If something goes wrong

“Could not load your Permission to Contact settings.”: The page couldn’t load your configuration. Refresh the page or try again in a moment.

“Cannot enable this form: {reasons}”: Your save was rejected because you tried to activate the form while blockers remained unresolved. The message lists the exact blocker codes that are still outstanding (for example AGENCY_LEGAL_NAME_REQUIRED, AGENT_NPN_REQUIRED) rather than the plain-language text shown in the checklist above. Match each code to its description in step 6, fix it, and save again. When this happens, the Activate this form checkbox reverts to off. Your last-saved state is unaffected.

“Could not save. Try again.”: A generic save failure unrelated to activation blockers. Try saving again in a moment.

You don’t share your PTC form’s link from this settings page. Open a post for editing, select Capture in the editor toolbar, and choose Permission to Contact as the link type in the dialog that opens. See Collect leads from your posts for the full flow. From there:

  • If your form isn’t activated yet, or blockers remain, the dialog tells you and links back to this settings page.
  • If every blocker is resolved but you haven’t checked Activate this form and saved yet, the dialog reads “Your Permission to Contact form is disabled. Enable it in Permission to Contact settings.”
  • If your form is activated and ready, the dialog shows the link so you can insert it into the post.

Plan gating

Getting a working, shareable link to your PTC form requires a paid plan. If your plan doesn’t include it, the “Add a link” dialog shows:

Permission to Contact is available on paid plans. Upgrade to unlock.

Select Upgrade to unlock to go to the Billing page. This settings page itself (filling in your agency details, coverage, and activation checklist) is not affected by this. Only getting a usable public link is gated.

What activating this form does not do

  • It does not add a way to revoke a signed permission. Once someone signs, there is no in-app action that reverses it.
  • It does not produce a complete printable version of the form. Some sections are designed to appear only when printed, but this has not been verified as a complete, ready-to-use paper workflow.
  • Activating this form does not mean your counsel has reviewed it or your carriers have approved it. That review happens outside the app. The banner at the top of this page is a permanent reminder, not a one-time notice you can dismiss once you’ve done it.

A signed permission is valid for 12 months from the date it’s signed. After that, it expires and the person needs to sign again before you rely on it. See View and export signed consents for how expiry shows up in your signed records.
